Athletic Forms

No student will be allowed to participate in any sport without completing all required athletic paperwork.

Physical Evaluation (FHSAA EL2)

A yearly physical examination is required for high school and middle school sports. The student shall submit the completed physical examination result, completely filled in and properly signed, attesting that he/she has been examined and found to be physically fit for athletic competition, to the appropriate school personnel, prior to becoming a member of any athletic squad or team. The form must be filled out, signed and notarized. Athletic physicals must remain up-to-date throughout the entire academic year, as they are valid for one year from the completion date.

Consent and Release (FHSAA EL3)

A student must have the consent of his/her parent(s) or legal guardian(s) to participate in interscholastic athletics at FSDB before participating in competition or engaging in any physical activity, like tryout, practice, workout, etc. The student and his/her parent(s) or legal guardian(s) must release the FHSAA, FSDB, and consent officials from all liability for any injury or claim that may result from the student’s participation in interscholastic athletics. The forms are valid for one year from the date the student and parent signed.
